SECTION 01

Start with one subscription

Use this worksheet when the details of a subscription are spread across a spreadsheet, checkout, inbox and support system. It gives your team a consistent place to note what is on file and where to retrieve it.

Pick one subscription you are authorized to review. Copy the example row, replace its fictional references and leave information unknown if your source does not establish it. Keep sensitive customer details and your working copy in your business’s approved storage.

SECTION 02

Keep a reference to the offer and agreement

Record the dated offer version, price, currency and billing interval. Keep a source reference to the terms that applied. The wording you save today may differ from what a subscriber saw earlier.

For the agreement, record the event time and source separately. A successful payment does not by itself establish agreement to renewal terms. If you cannot find a supported offer-to-agreement link, mark it unknown and assign a person to review it.

SECTION 03

Separate the notice plan from the result

The worksheet includes the notice category, planned date, actual send time, message reference and reported delivery result. These fields describe your records; they do not calculate a legal deadline or send a message.

Use the result the source actually reports. Sent, accepted, delivered and opened describe different events. If you only have a staff summary, label it as a summary and retain the original message when available.

SECTION 04

Keep two cancellation dates

A customer can ask to cancel before the billing system marks the subscription ended. Record the request time and channel separately from the effective end date. Keep a reference to the relevant ticket or provider event.

Use the open-question and review-owner columns to make the next action clear. The worksheet does not score legal compliance or prove that an absent record never existed.

SECTION 05

Use the worksheet or move the record into a workspace

You can keep using this worksheet by itself. When multiple staff members need linked records and customer histories, TermWitness provides manual customer, offer, subscription, notice and evidence records plus CSV exports.

This evidence worksheet is a review aid, not a file that can be imported directly into every TermWitness record type. The current customer import accepts CSV or supported Excel files with mapped name, email, plan label and renewal-date columns. Provider synchronization and subscriber notice delivery are not available.
